If you require a replacement Membership Certificate for any reason, you must complete the Replacement Certificate Application Form, which can be downloaded from the 'Related documents' box (bottom right).
This must be returned with a cheque payment for £37. (Please write your name and membership number on the back of the cheque). If you would like to pay by bank transfer, please contact the Finance Department.
A photocopy of your passport/EU ID card will also be required.
Members are reminded that if an RCVS Membership Certificate is mislaid, it could be used for fraudulent purposes.
If you are a Veterinary Nurse and have lost your RCVS Membership Certificate, please contact the Veterinary Nursing Department.
