Replacement Membership Certificates

If you require a replacement Membership Certificate for any reason, you must complete the application form (PDF 120Kb).  This must be returned with a cheque or completed credit card form for £35. (Please write your name and Membership Number on the back of the cheque).

A photocopy of your passport/EU ID card will also be required.

Members are reminded that if an RCVS Membership Certificate is mislaid, it could be used for fraudulant purposes.
